This is Vermont's 2006 111-Page Olmstead Plan. Vermont also has a Vermont Olmstead Commission. This highlights the problem of what happens -- even if one has a relatively good document -- if it is NOT subject to constant modification and improvement -- it can get out of date pretty quickly.
